Forms and Certification

Find the forms and resources needed to maintain or obtain your certifications. Whether you’re pursuing Medical Gas or Welding credentials, the information below will guide you through requirements, continuity forms, and how to submit completed paperwork.

 

Medical Gas Certification

Medical Gas systems personnel must document their qualifications through written and practical testing. Certification requires:

  • Four years of documented experience in piping system installation
  • Completion of a minimum 32-hour training course
  • Passing a multiple-choice exam covering ASSE Standard 6000, NFPA 99, and NFPA 55 (77% passing score)
  • Passing a proctored practical brazing exam

Certification is valid for three years, as long as brazing continuity is maintained.

Welding Certification

Local Union 441 members and traveling UA members can participate in welding testing by contacting the PPATKS office at least one week before the testing date. If you plan to test on materials other than 2” Sch. 80, 2” XXS, or the super coupon, please contact the office at least 10 days in advance to ensure the necessary materials are available. Key points:

  • Space is limited; scheduling is on a first-call basis
  • If fewer than four participants are registered, the test may be canceled
  • This is a formal test; participants should be fully prepared
